Participating Home Health Agencies

As of April 23, 2007, a total of 4,960 home health agencies have signed up for the HHQI National Campaign.

Open Enrollment
All Medicare-certified home health agencies (HHAs) can join the campaign at any time. Almost 5,000 HHAs have already joined. What other agencies would want to join now?

  • HHAs that just became aware of the HHQI Campaign
  • HHAs that were not sure what was involved
  • HHAs that just received their Medicare certification.

Best Practice Intervention Package
   

The Best Practice Intervention Packages will remain posted until February 29, 2008.

Best Practice Intervention Package: Medication Management

The medication management package offers information to assist clinicians with improving patient and caregiver medication management. The focus is on quality improvement, including assessment and interventions.

Featured in the package is the “Be Safe & Take” Clinician Enrichment Program. This program is designed for nurses and therapists. The program includes the following:

  • Pre and post medication competency test
  • Educational materials
  • National medication improvement tools.

The nurse track, which includes the “Be Safe & Take” Clinician Enrichment Program, offers 2.0 FREE contact hours for registered nurses.

Fast Track

A Fast Track Package – Medication Management is also available.

All packages or tracks are available on the HHQI Web site – www.homehealthquality.org.

Medication Management WebEx

A 15-minute WebEx on the Medication Management Best Practice Intervention Package will be conducted. It will include an introduction and information highlighting new resources available with this package. The WebEx will provide a quick overview of the package.

Emergency Care Planning BPIP Survey

Summary of Results
The survey results from the Emergency Care Planning BPIP are now available on the HHQI Web site.

The Best Practice Intervention Package Survey helps the HHQI team

  • improve the packages
  • understand barriers to accessing the packages, etc.
  • know whether we are on target with the materials
  • recognize when there is a need modify the materials.

Thanks to everyone who completed the survey. The next survey will be available May 8 – May 15 and will focus on the Medication Management BPIP.

Judy Lentz is the Chief Executive Director for the Hospice and Palliative Nurses Association (HPNA). This organization is for individual members of the nursing team working in the specialty of hospice and palliative care across the life span continuum.

HPNA pledges to:

  • Serve as a monthly consultant to the home health QIOSC with completion of the monthly campaign intervention packages (providing material for the Hospice/Palliative Connection)
  • Share HPNA patient-centered multidisciplinary tools/resources and tips that can assist home care agencies with increasing quality of care and decreasing avoidable hospitalizations
  • Make a variety of educational multidisciplinary programs/products available to home care via a variety of methods
  • Serve as a research resource; agencies can contact hpna@hpna.org for support.
